1) Physician records
2) Imaging such as CT scans, mammogram, colonoscopy. (See below.)
3) Pathology report 
4) List of medications
5) Registration forms
6) Health Questionnaires

Second Opinions

We provide second opinions. If insurance is available it will be billed; if not, a fee for service will be charged in advance. Please check with the staff.  We will need all the information for a new patient visit at the time of consultation. Click here for more information about the cost of a second opinion.

Diagnostic Imaging

Ask the imaging providers listed below for a free copy of your imaging on a CD. Because the copy is for Dr Singhal, they should give you the CD for free.  Bring the CD with you on your first visit to our office.  Please bring mammograms with you to your first appointment.
